I just filled out the form and got my request back. However, I then started second guessing how I filled it out. I put my name and as the name and CIV as the grade. I am a dependent flying on Command Sponsorship. Should by husbands info been put there and my info below? or did I do it right?

Just a little apprehensive about this whole Space A thing!

No problem; you did it correctly. It's the traveler's information that gets entered into the form, not the sponsor's. (One very small thing: instead of putting "Civ", it would've been more correct to select either "Enl Dep" or "Off Dep", but that can be corrected at the terminal, so don't worry about it.)
